Chapter 3

Xiao Zhen Jie

The acupuncture theory is easy to talk, but difficult to comprehension and do it good. The normal doctor just know the local needling skills for treatment. The high level doctor will know the patient’s Qi and blood excess and deficiency conditions to use tonify and sedate techniques to do the treatment. The patient’s body Qi has a fighting with the sick Qi, the sick Qi will follow the body Qi’s door way come into patient’s body, if the doctor can not find out which door way the sick Qi come in, the doctor will not find out use which points to treat and what is the true reason of the illness, and can not make treatment principles. The secret of acupuncture is correct use fast and slow speed to needling, the normal acupuncturists just know needling the local limbs points to do the treatment, they don’t know how to figure out the patient’s body Qi and blood changing condition of fighting the sick Qi. The high level doctor will know to figure out the patient’s body Qi changing condition, the Qi changing condition will show out at the shu points, the doctors understand the condition of Qi’s changing, they can correct use fast and slow needling techniques to tonify or sedate the patient body Qi to do the treatment. The changing Qi in the points is very tiny, during needling “de Qi”, then meditation hold the Qi to detention the needle don’t let the Qi go, if the meridian Qi come, should not counter it direction to sedate it, if the meridian Qi is excess, should not use tonify way to needling; if the meridian Qi go, should not follow the direction to tonify it, if the meridian Qi is deficiency, should not use sedate way to needling; the Qi’s changing is very fast, acupuncture like archery, if you aim the target, you should not delay the chance, if you not aim good the target, you should not shoot out the arrow, if the doctor don’t know how the tonify and sedate the Qi, it will run out the patient’s body Qi and blood, and can not dispel the sick Qi. So, the doctor must know the Qi’s come and go, must know the Qi’s direction of excess and deficiency. Also know the time of the Qi changing and know the chance to needling the Qi. The normal acupuncturist don’t know the patient body’s Qi and blood changing condition secret. Only the high level doctor know the meaning of needling. The meridian Qi go is inverse condition, because the Qi go, the meridian become deficiency, so this is inverse condition. The meridian Qi come is along condition, because the Qi come, the meridian become normal condition, so this is along condition. If the doctors understand the meridian Qi excess or deficiency and the meridian Qi inverse or along condition, so they can correctly use the needling techniques without problem, and know the points location they want needling. counter meridian Qi direction to needling is sedate, follow meridian Qi direction to needling is tonify. “Xu Ze Shi Zhi” means cun location pulse deficiency, should use tonify needling way to strong body Qi; “Man Ze Xie Zhi” means if the cun location pulse full, should use sedate needling way; “Wan Chen Ze Chu Zhi” means if the meridian has blood stasis, should use bleeding needling way to dispel the blood stasis; “Xie Sheng Ze Xu Zhi” means if the sick Qi excess, should use sedate way to dispel it; “Xu Er Ji Ze Shi” means slow insert needle fast pull out needle is tonify way; “Ji Er Xu Ze Xu” means fast insert needle and slow pull out needle is sedate way; “Yan Shi Yu Xu, Ruo You Ruo Wu” means after tonify needling, the patient’s body Qi will stronger, after sedate needling, the patient’s sick Qi will be dispelled; “Cha Hou Yu Xian, Ruo Cun Ruo Wang” means check out the meridian Qi deficiency or excess and the illness syndromes acute or chronic, to decide first tonify body Qi and sedate sick Qi lately, or first sedate sick Qi and tonfiy body Qi lately, to build up body Qi and dispel sick Qi, and the same time based on the meridian Qi condition to decide use which needling way; “Wei Xu Yu Shi, Ruo De Ruo Shi” means the tonify way can make the patient feel body Qi stronger, the sedate way can dispel patient sick Qi and feel comfortable. “Fu Qi Zhi Zai Mai Ye, Xie Qi Zai Shang” means during the sick Qi attack body meridians, the wind cold or wind heat will stay in upper part of body, because wind is light energy will go up. “Zhuo Qi Zai Zhong” means the damp cold or damp heat energy will stay on middle part of body, because the food and water energy in the stomach and intestine for digestion, the essence will go up to lung, and the rest of food and water will stay in stomach and intestine, if the irregular diet cause damp cold and damp heat energy unbalance, so will cause digestion system get sick energy make the dampness stay in stomach and intestine the middle part of body. “Qing Qi Zai Xia” means the damp cold energy will stay in the lower part of body, mostly attack feet and legs from the earth. “Zhen Xian Mai Ze Xie Qi Chu” means needling the upper part of body meridians’s points, the wind sick energy will be dispelled. “Zhen Zhong Mai Ze Xie Qi Chu” means needling the middle jiao meridians such as yang ming meridians He sea points, the dampness energy will be dispelled. “Zhen Tai Shen, Ze Xie Qi Fan Chen” means if needling too deep, the sick Qi will go deep inside body. “Pi Rou Jin Mai, Ge You Suo Chu” means the skin, muscle, tendon and meridian, each part of them must connect by different Jing Luo, so their sick Qi can be treated by different Jing Luo. “Qu Wu Mai Zhe Si” means the patient sick in five Zang and very deficiency of Zang Qi, if the doctor use sedate needling the five Zang’s shu points, it will cause run out five Zang Qi and the patient will die. “Qu San Mai Zhe Kuang” means if the doctor can not figure out the deficiency or excess condition and mistake use sedate needling the hand and foot three yang meridians shu points, will cause the patient loose too much three yang meridians Qi, and body energy will too deficiency to recover. “Duo Yin Zhe Si” means the Yin Qi of five Zang come out at Li13 and Lv10, if the doctor sedate five times those points, will cause patient body Zang Yin Qi run out and die. “Duo Yang Zhe Kuang” means if doctor sedate out the three yang meridians’ Qi, will cause the patient’s Shen deficiency and cause maniac. Above is talking about the avoid of needling. “Du Qi Se, Cha Qi Mu, Zhi Qi San Fu, Yi Qi Xing, Ting Qi Dong Jing” means the high level doctor will use eyes to check out the patient’s five colors, and feeling the pulse full or weak, rapid or slow, slippery or hesitant, and whole body conditions, to detail diagnosis the patient’s illness. “Zhi Qi Xie Zheng” means the doctor will figure out the patient’s illness caused by excess or deficiency conditions which the fighting of body Qi with sick Qi. “You Zhu Tui Zhi, Zuo Chi Er Yu Zhi” means during needling, use right hand push insert needle, use left hand protect the needle, and use different techniques to push and pull needling. “Qi Zhi Er Qu Zhi” means after insert needle and “De Qi”, use sedate or tonify techniques to balance the Qi function and then can pull out needle. “Tiao Qi Zai Yu Shi Zhong Yi” means during needling should concentrate and focus on needling. “Jie Zhi Jiao San Bai Liu Shi Wu Hui” means whole body meridians Qi and blood flow into 365 points on different part of body. “Wu Zang Zhi Qi, Yi Jue Yu Nei” means five Zang Jing Qi very deficiency even almost empty. During this condition, the patient’s cun pulse is very weak and without root, lower pressure of the pulse even can not feel it, it should tonify the patient’s Yin Jing, should not tonify Yang Qi, but if the doctor mistake tonify Yang Qi at yang meridians’ He sea points and detention needle to gather the Yang Qi, because the patient Yin Jing very deficiency can not hold Yang Qi, and too much Yang Qi come will run out the patient’s Yin Jing, cause five Zang Yin Jing seriously empty, and the patient will die, die without Yin Jing to hold Yang Qi, Yang Qi is not root to action, so the patient died in quiet. “Wu Zang Zhi Qi Yi Jue Yu Wai” means five Zang Jing Qi is very weak and can not feel it from upper pressure, so the cun pulse shows very deep and weak, this condition is five Zang six Fu Yang Qi almost run out. This condition should tonify Yang Qi, but if the doctor mistake use the tip of limbs shu points to needling and tonify the Yin Qi of five Zang, and detention needle to gather the yin Qi, the yin Qi will stronger than yang Qi cause yang Qi more deficiency, and then the yin cold energy will from tip of limbs go to center of body(Jue Ni), and the patient will die. During the dead, because yin and yang energy fighting, and yin Qi is stronger than yang Qi, so the patient will maniac syndromes happens. Check the patient’s eyes, will find the Shen is very strong, because five Zang and six Fu Jing Qi all go into the eyes; and the face five Zang color is very brightening shining, and at the same time, because the yin energy strong, the patient’s mouth speak voice will stronger and bigger than normal time.
小針解第三

所謂易陳者,易言也。難入者,難著於人也。粗守形者,守刺法也。上守神者,守人之血氣有餘不足可補瀉也。神客者,正邪共會也。神者,正氣也,客者邪氣也。在門者,邪循正氣之所出入也。未睹其疾者,先知邪正何經之疾也。惡知其原者,先知何經之病所取之處也。

刺之微在數遲者,徐疾之意也。粗守關者,守四支而不知血氣正邪之往來也。上守機者,知守氣也。機之動不離其空中者,知氣之虛實,用針之徐疾也。空中之機,清靜以微者,針以得氣,密意守氣勿失也。其來不可逢者,氣盛不可補也。其往不可追者,氣虛不可瀉也。不可掛以發者,言氣易失也。扣之不發者,言不知補瀉之意也。血氣已盡而氣不下也。知其往來者,知氣之逆順盛虛也。要與之期者,知氣之可取之時也。

粗之闇者,冥冥不知氣之微密也。妙哉!工獨有之者,盡知針意也。往者為逆者,言氣之虛而小,小者逆也。來者為順者,言形氣之平,平者順也。明知逆順正行無問者,言知所取之處也。迎而奪之者,瀉也;追而濟之者,補也。

所謂虛則實之者,氣口虛而當補之也。滿則泄之者,氣口盛而當瀉之也。宛陳則除之者,去血脈也。邪勝則虛之者,言諸經有盛者,皆瀉其邪也。徐而疾則實者,言徐內而疾出也。疾而徐則虛者,言疾內而徐出也。言實與虛若有若無者,言實者有氣,虛者無氣也。察後與先若亡若存者,言氣之虛實,補瀉之先後也,察其氣之已下與常存也。為虛為實,若得若失者,言補者佖然若有得也,瀉則恍然若有失也。

夫氣之在脈也,邪氣在上者,言邪氣之中人也高,故邪氣在上也。濁氣在中者,言水谷皆入於胃,其精氣上注於肺,濁溜於腸胃,言寒溫不適,飲食不節,而病生於腸胃,故命曰濁氣在中也。清氣在下者,言清濕地氣之中人也,必從足始,故曰清氣在下也。針陷脈,則邪氣出者取之上,針中脈則濁氣出者,取之陽明合也。針太深則邪氣反沉者,言淺浮之病,不欲深刺也。深則邪氣從之入,故曰反沉也。皮肉筋脈各有所處者,言經絡各有所主也。

取五脈者死,言病在中氣不足,但用針盡大瀉其諸陰之脈也。取三陽之脈者,唯言盡瀉三陽之氣,令病人恇然不復也。奪陰者死,言取尺之五里五往者也。奪陽者狂,正言也。睹其色,察其目,知其散復,一其形,聽其動靜者,言上工知相五色於目。有知調尺寸小大緩急滑澀以言所病也。知其邪正者,知論虛邪與正邪之風也。

右主推之,左持而御之者,言持針而出入也。氣至而去之者,言補瀉氣調而去之也。調氣在於終始一者,持心也。節之交三百六十五會者,絡脈之滲灌諸節者也。

所謂五臟之氣,已絕於內者,脈口氣內絕不至,反取其外之病處,與陽經之合,有留針以致陽氣,陽氣至則內重竭,重竭則死矣。其死也,無氣以動,故靜。

所謂五臟之氣,已絕於外者,脈口氣外絕不至,反取其四末之輸,有留針以致其陰氣,陰氣至則陽氣反入,入則逆,逆則死矣。其死也,陰氣有餘,故躁。所以察其目者,五臟使五色循明。循明則聲章。聲章者,則言聲與平生異也。
